This is an in-office, face to face visit for either acute or chronic medical conditions. This includes up to 2 in-office testing (u/a, urine pregnancy, finger stick blood glucose, A1c, etc) and other labs are available for additional costs. Any labs required can be sent out to an outside lab and billed to your insurance or can be charged self-pay prices.

Initial Concierge Medicine In-Office Appointment

1 hour @ $199.00

This is your initial appointment after your membership has been approved. This visit includes a complete review of history medications reviewed, physical exam if necessary, and a treatment plan formulated. Basic labs will be included in the cost. We request that you bring all of your medications, your blood pressure cuff to be tested against ours for accuracy, your list of doctors/specialists, and your questions. We look forward to starting on your journey to Abundant Life.

In-Office New Patient Physical

1 hour @ $199.00

This is an in-office, face to face visit for new patients for a physical with labs. If you need forms completed for employment, life insurance, physicals, etc, please provide them prior to the visit. This includes up to 2 in-office testing (u/a, urine pregnancy, finger stick blood glucose, A1c, etc) and other labs are available for additional costs.

Independent Medical Examination New Client

2 hours 30 minutes @ $1,499.00

We will review records, do an exam and provide a medical opinion writeup (typically used for retirement, disability clearance or evaluation with form completion and/or letter)

Pre-employment physical (non-member)

30 minutes @ $199.00

Do you need a form completed for a pre-employment physical. This appointment comes with a medical history review, examination, completion of paperwork, and any labs that need to be done.

Veteran's Medical Treatment

30 minutes @ $199.00

If you are a Veteran needing medical assessment, diagnosis, and treatment, you can be seen for a cash pay price through Abundant Life Concierge. We do not take insurance at this time, this is a cash pay fee for service with 15% off Veteran's discount

In-Office Urgent Care (non-member)

20 minutes @ $129.00

In-office visit (same day or next day service based on availability) for acute conditions like headaches, joint or muscle pain, UTI/urinary complaints, skin conditions, genitourinary problems/conditions, mental health conditions, cough/cold/congestion/allergies, etc. Up to 2 in-office testing included in visit (fingerstick glucose, urine pregnancy test, urinalysis, rapid strep), other testing including lab sendouts will incur other costs)

Chronic Condition Management Telemedicine Visit (non-member)

30 minutes @ $99.00

This visit is for management of chronic medical conditions and refills. Conditions include, but not limited to, hypertension, diabetes, high cholesterol, asthma, seizures, migraines, acid reflux, IBS, mental health med management (no controlled substances), and some cardiac conditions like arterial disease medication management. Requires calibration of home monitoring devices (bp cuff and glucometer) at least once annually.

Form Completion and Correspondence (non-member)

30 minutes @ $249.99

Do you need a letter written or forms completed? I can review your medical records (records can be uploaded to my HIPAA compliant portal) and write your letter or correspondence. Examples: letters to break contracts for gym membership, letter certifying medical conditions and prognosis, medical forms for loan deferment, etc.

In-Office Visit Followup

30 minutes @ $149.00

Medication Refill Request (NOT AN IN-PERSON OR VIRTUAL APPOINTMENT)

10 minutes @ $39.00

If you are out of your chronic non-narcotic/not controlled substance medication and need an emergent refill until you can get in to see your doctor (or be established as a new patient with Abundant Life or another practice), you can upload a picture of your medication bottle and a refill up to 90 days can be sent to the pharmacy of your choice. This is ideal for out of town guests or people not established with a doctor yet but need refills. PLEASE NOTE, THERE IS NO ACTUAL APPOINTMENT

Sports Physical

20 minutes @ $25.00

Sports/Camp physical forms completed by a board certified Physician for clearance to play sports or participate at camp! A general sports physical form will be available, but if the team requires a specific form, please bring it to the appointment (for example: Pop Warner, or Boy Scouts, etc).

Independent Medical Examination Repeat Client

2 hours @ $999.00

Second Opinion Consultation

30 minutes @ $349.00

Do you have lab results, XR or radiology results, medical notes, or medical condition diagnoses that you want a second opinion on? This service provides the opportunity for another doctor to review your history and notes and give a second opinion.

Acute condition Telemedicine Appointment

10 minutes @ $89.00

Telemedicine visits occur through a website or app on your computer or mobile device and are available to address acute medical conditions.
Appropriateness of visit will be determined at the onset of the appointment. This is typically acute issues like cough, cold, ear ache, sore throat, headache, ear pain, allergies, urinary symptoms, minor injuries, discharge, minor aches and pains, abdominal pain, muscle strain, etc.
